DOCUMENTS FOR STUDENTS

- This PDF has all of the information included in the Student Teaching Handbook. Please review the entire document to see what is expected of you and your cooperating teacher during your student teaching placement. After reviewing the PDF of the Student Teaching Handbook, follow the link to the eForm to acknowledge you have read it.

- Review this document to understand what the professional expectations and logistical requirements are for your student teaching placement. After you have reviewed this document, click on the eForm to acknowledge that you have read and understand these requirements and expectations.

- Student Teaching Log – Print, Scan/Photo & Upload Below
- Print out the student teaching log to record your student teaching hours. This log must be signed by both your cooperating teacher and your Pratt faculty member and at the end of the semester, a completed log must be scanned and uploaded using the upload button below

DOCUMENTS FOR COOPERATING TEACHERS
Your cooperating teacher is required to complete the following online documents. Use the links below to download copies of the documents for your cooperating teacher to read, print, sign and/or upload as appropriate. You can then upload the documents for them at any time on the ADE eForm site using the UPLOAD BUTTON BELOW.
- Student Teaching Handbook – View Handbook (PDF) & Confirm Read (eForm)
- Point your cooperating teacher to the following documents in the Student Teaching Handbook: To the Cooperating Teacher; Rubric; Explication of Terms/Glossary. After you and your cooperating teacher have reviewed the PDF of the Student Teaching Handbook, particularly the pages identified above, follow the link to the eForm to acknowledge that you have reviewed it with your cooperating teacher.
- Memorandum of Understanding – Print, Sign, Scan/Photo & Upload Below
- Print or photocopy the Memorandum of Understanding (p. 21) for your cooperating teacher. Your cooperating teacher is required to review and sign the document to acknowledge that they understand the requirements of your student teaching placement. Once the document has been signed by your cooperating teacher, scan/photo the document and upload it to the ADE eForm site using our UPLOAD BUTTON.
