Completed Health Evaluation Packets must be uploaded January 1 for Spring entrants, June 1 for Summer entrants, and June 15 for Fall entrants.

New York State requires that all students born on or after January 1, 1957 must prove immunity to Measles, Mumps, and Rubella. Students must also review information regarding the Meningococcal Meningitis vaccine and provide proof of vaccination or a signed refusal. Pratt requires that all students complete a tuberculosis screening and a physical examination. If you have lived in a country with a high incidence of active tuberculosis, you will need to undergo additional testing. You can find a list of high incidence countries here.
The Health Evaluation Packet consists of three forms that must be completed by a medical healthcare provider.
- Immunization Records
- Tuberculosis Screening
- Physical Examination
Download and print out the packet and bring it with you to your medical provider’s office. Once you and your medical provider have completed all three parts, scan or photograph each part and upload them to the Pratt website.
