Immunization Information

Completed Health Evaluation Packets must be uploaded January 1 for Spring entrants, June 1 for Summer entrants, and June 15 for Fall entrants.

Step 1: Download and print out the Health Evaluation Packet

The health packet consists of three separate forms (Immunization Records, Tuberculosis Screening, and Physical Examination) that must be brought to and completed by a medical healthcare provider.

  1. Immunization Records: New York State requires that all students born on or after January 1, 1957 must prove immunity to Measles, Mumps, and Rubella. Students must also review information regarding the Meningococcal Meningitis vaccine and provide proof of vaccination or a signed refusal.   2. Tuberculosis Screening. Pratt requires that all students complete a tuberculosis screening within the last 12 months. 
  3. Physical Examination. Pratt requires that all students complete a physical examination in the last 6 months. 

Step 2: Upload a scan or photograph of your completed Health Evaluation Packet once you and your medical provider have completed all three parts (Immunization Records, Tuberculosis Screening, and Physical Examination), to the Pratt website.

FOR COVID-19 VACCINATIONS: Effective May 11, 2023, proof of COVID-19 vaccinations, boosters, and exemptions is no longer required for new or current Pratt students, faculty, or staff, and will no longer be a determinator for Pratt ID access. Pratt strongly encourages all community members to remain up-to-date with the latest COVID-19 vaccines and boosters. Pratt will continue to follow guidance from the CDC, as well as city, state, and federal directives, and may revise policies if circumstances require..

Download and print out the packet and bring it with you to your medical provider’s office. Once you and your medical provider have completed all three parts, scan or photograph each part and upload them to the Pratt website.
