I-20 Request Forms

Fall 2012 & Spring 2013 CEP Students

If you are applying for the Fall 2012 or Spring 2013 full-time Certificate in English Proficiency (CEP) Program, you may download and fill out the 2012-2013 CEP I-20 Request Form. Please submit this form after you have filled out the online CEP Application. 

 

Summer 2012 SCP Students

If you plan to apply for Pratt's 2012 Summer Certificate in English Proficiency (SCP) Program, please submit your SCP Application form as soon as possible. If you have been accepted for the Fall 2012 semester please note this on your application form. Once you have been accepted you will receive instructions on how to access the I-20 Request Form online.
