I-20 Request Forms

Fall 2013 & Spring 2014 CEP Students

If you are applying for the Fall 2013 or Spring 2014 full-time Certificate in English Proficiency (CEP) Program, you may download and fill out the 2013-2014 CEP I-20 Request Form. Please submit this form after you have filled out the online CEP Application.

Summer 2013 SCP Students

If you plan to apply for Pratt's 2012 Summer Certificate in English Proficiency (SCP) Program, please submit your SCP Application form as soon as possible. If you have been accepted for the Fall 2013 semester please note this on your application form. Once you have been accepted you will receive instructions on how to access the I-20 Request Form online.
